Sowe Valley Primary Care Network (PCN) is a collaboration of eleven GP practices in the East of Coventry with 86,000 registered patients. Our practices are all rated ‘good’ to ‘outstanding’ by the CQC and we have come together through the national Primary Care Networks initiative to innovate and develop new models of primary care.
We are now seeking applications from experienced physiotherapists to join our team as we roll out a First Contact Physiotherapist (FCP) service across our Primary Care Network.
As part of the FCP team you would be responsible for delivering this innovative model of care within primary care using a combined hub and in-practice model. This model of care combines an in-practice level triage of routine MSK problems and FCP work. On this basis a knowledge of secondary care consultant clinic work or triage experience is essential. The successful candidate will be expected to provide frontline expert screening, assessment and diagnosis of Musculoskeletal conditions to ensure patients are supported to self-manage or receive support from the most appropriate clinical pathway. Patients will be streamed by GP reception staff to the FCP clinic directly.
The clinician would also be expected to broaden their skillset by optimising holistic input for patients in line with the role of FCP. Enhancement of these skills will be provided by a nominated GP mentor and our team of clinical pharmacists.
You will be required to work closely with the Primary Care Network clinical lead to develop the service and audit activity. You will be expected to develop links with colleagues within the service, local GPs and secondary care colleagues to ensure delivery of integrated, patient focused care.
We are looking for an enthusiastic physiotherapist working at an advanced level looking to develop their role in a supportive, innovative environment. You must have experience of triage, the ability to request and interpret imaging and qualifications / evidence to support this.
MSc, prescribing qualification or injecting diploma desirable.

Salary is based on AFC band 7-8A dependent on experience with flexible hours of work between 8am - 6pm Mondays - Saturdays. Full and part-time working will be considered.
