Health Partners is growing, and we now have a fantastic opportunity for experienced Clinical Case Managers to join our team of occupational health professionals.

About you:

  • You are a qualified registered healthcare professional;
  • You are either NMC Part 1 or 3 registered or a HCPC-registered Physiotherapist or Occupational Therapist;
  • You are experienced in case management and the provision of Occupational Health advice;
  • You have experience in a commercial occupational health setting, preferably with exposure to case management and health surveillance;
  • You have excellent knowledge of occupational health and employment legislation.

About the position:

As a Clinical Case Manager, you will be an integral part of our services to clients working within company service-level agreements, coaching their managers to enable employees to return to good health and work if they have been off with ill health and to maintain/improve their health and wellbeing.

The work is flexible, based in your home, and focusses on delivering services to our clients, with a mix of 20- to 45-minute appointments throughout the day.

As a Clinical Case Manager, you will support the triage function of the business unit, participate in case conferences with the client and provide evidence-based case management services for referrals relating to attendance and fitness for work, as well as other requests for Occupational Health advice.

You will be part of an experienced and caring team of nurses, advisors and support staff who will provide you with continual and consistent support, guidance and development. You will be not be alone.
