The aim of 'My plan for life' is to help people with chronic conditions to think about what matters most to them and what they want from their care – including returning to work. The message to patients is: you are the expert on how your condition makes you feel. If you can think about what is important to you, you will be able to ask the right questions and get the right information and support.
Patients can use the guide to note down questions that they want to ask and any advice they've had. They can also use it to keep the contact details for the different members of their healthcare team in one place. It has space to record appointments, test results and discussions too.
Organisations such as Arthritis Care, Arthritis and Musculoskeletal Alliance, National Ankylosing Spondylitis Society and National Rheumatoid Arthritis Society are promoting the guide, which is now being piloted in different regions of the UK.
