Phyllis Tuckwell Hospice Care is an innovative independent provider of Specialist Palliative Care based in Farnham and Guildford serving a population of 550,000 across West Surrey and North East Hampshire.There is a large multi-disciplinary Care at Home service including a Hospice Care at Home, plus an 18 bed In-Patient Unit, and a Living Well multidisciplinary team providing a comprehensive living well programme. We work closely with community services teams, the 2 neighbouring acute trusts and 5 community hospitals in the catchment area.

Therapy Lead & Living Well Services Manager (Nursing & Therapies)

Salary £46,453.00 per annum

37.5hrs per week  

Phyllis Tuckwell is seeking an experienced HCPC ‘Therapy Lead & Living Well Services Manager’ (Nursing & Therapists) to provide clinical leadership and management for the team. The role involves being a Therapy Lead for therapists working in the In-Patient Unit, Community and Living Well, as well as the Living Well Manager for the nurses and therapists. The role involves a joint approach with the Living Well Services Manager (Patient & Family Support), developing innovative multidisciplinary Living Well Programmes to enhance coping skills and strategies for patients with advanced illness and carer support. Experience of change management, workforce development, practice and service development as well as clinical governance and partnership working is essential. The post holder will be a member of the Senior Clinical team and be expected to be part of an on call out of hour’s rota for advice alongside other Managers.
