An exciting opportunity has arisen to get involved with the newly commissioned Community Care bed model in Leeds. The new service is a joint venture with our Adult Social care partners.

The admission into the beds will be on two pathways, Discharge to Assess or an Intermediate care pathway. The bed bases will be recovery hubs in South, North West and East of the City.

Our Multi-Disciplinary Teams will deliver daily evidence-based nursing and therapeutic interventions, specifically designed to support people reach their maximum independent state, using therapy outcomes measures to demonstrate our success in achieving this. Our ethos is that the rehabilitation, recovery and reablement of service users is everybody's business.

The Role

We require an experienced Physiotherapist who is looking for a challenging leadership role, providing a high quality physiotherapy service within our skilled and forward thinking multi-disciplinary team. 

We provide Therapy interventions between the hours of 8am and 8pm, 7 days a week, your role will be to work with colleagues to assess and provide Physiotherapy interventions and advice as required. You will work as part of a multidisciplinary team with registered nursing, Occupational therapy, technical instructors and you will support the Care home managers and support staff
