Activate your Heart
This is an alternative to conventional centre-based programmes. It allows patients to remotely self-manage their rehabilitation. It includes:
- A comprehensive range of educational resources
- Advice and support to make lifestyle changes and decrease risk factors
- Interactive – you monitor your patient's progress and outcomes
It allows users to:
- Keep an exercise diary
- Monitor their weight and stress levels
- Create personal goals
- Test their knowledge
- “Ask the Expert”
You can access the programmes from a computer, tablet or smartphone.
A free 3-month trial is available to all cardiac rehabilitation teams. This includes access to the programme and also support and training to get you started.

The Heart Manual
NHS Lothian's individualised home-based, supported self-management programme is for patients recovering from acute myocardial infarction to revascularisation.
They're offering free remote training and digital versions for cardiac rehab teams until 30June 2020. Contact heart.manual@nhslothian.scot.nhs.uk for more information.
